Emergency Shelter
All eligible clients are initially admitted to the Center for 7 days, during which they are expected to meet with a Case Manager to create an Action Plan (described below). The client(s) and case manager will review the individual or family's unique circumstances weekly to determine if more time in the shelter is appropriate, up to a maximum of 90 days.
Transitional Living
The Anacortes Family Center reserves two units for Transitional Housing for qualified individuals or families. Transitional Housing clients can remain in the Center for up to 24 months and continue to receive case management and other supportive services.
Case Management
Each individual or family will be assigned a Case Manager who will work with the clients to identify strengths and needs and will collaborate with them to create a unique, personalized Action Plan. Each Action Plan will consist of short and long-term goals, including permanent housing, employment/income, education and any other specific goals identified by the clients and the case manager. Outlined in the Action Plan will be specific steps to reach the goals, including ways to build targeted skills and fill unmet needs. As a part of the stay at the center, the Case Manager will meet with each individual or family on a schedule determined by the Case Manager to assess the progress toward the outlined goals and to offer support and encouragement as necessary.
Supportive Services Through Referral
Medical Care
The Case Manager assists each client to find a primary care provider if they do not already have one so that they can receive medical treatment as needed and on a preventive basis. If the individual or family lacks insurance, the Case Manager can help them to access resources to obtain coverage.
Mental Health / Drug and Alcohol Services
Upon Intake, the Case Manager conducts a brief mental health and drug and alcohol assessment and may recommend certain services. A healthy, clear mind is the foundation for success and we want to ensure that all of our clients have every possible opportunity to succeed. Clients have the option to refuse mental health or drug and alcohol services, however if staff believes that anyone's behavior may jeopardize the safety or success of other residents, the Center may require individuals to participate in services to obtain or continue residency.
Program Requirements
Housing Search
While we are able to provide emergency housing, we are not a long-term housing solution. Therefore, it is imperative that clients continue to search for housing while staying at the Center. The Case Manager will work with all clients to identify housing options and assist them to obtain services.
Financial Management and Participation
Each client will create a budget outlining all income and expenses to help determine the best housing and living options. Clients are encouraged to put aside money to pay for security deposit, rent, etc.
Case Management / Action Plans
Each client / family will create an action plan within the seven (7) days as a requirement of being a resident in the Center. Participation in case management is required and each week you must be able to show progress towards case management goals to continue receiving services.
